BAILEYS PRAIRIE  Village Hall | 10AM-4PM    DECEMBER 20, 2017    

United Way of Brazoria County Disaster Assistance Coordinators will be traveling north, south, east & west to connect with residents who have unmet needs following Hurricane Harvey.  Coordinators will be at the Bailey’s Prairie Village Hall, 1680 Jimmy Phillips Blvd from 10 AM – 4 PM.

There will be limited appointments daily at each location, based on the number of coordinators available. Residents will be seen on a first come, first served basis.  The goal is to provide assistance after all other resources have been exhausted.

[ ] Most current utility bill (water and/or electric)
[ ] Current lease agreement (all members of the household must be listed, if applicable)
[ ] Valid DL, TX ID, Passport, or Military ID (for all household members over 18 years old)
[ ] Current proof of income (month of pay stubs, award letter(s), child support, unemployment, SSI, etc.)
[ ] Proof of FEMA registration (Explanation of Benefits)
[ ] All FEMA, SBA, & Insurance correspondence
* The following documents are required as they are applicable to your specific unmet needs request:
[ ] Three bids for appliances (for example: stove, refrigerator, washer, dryer, etc.)
[ ] Three bids for home repair (must be on contractor’s company letterhead)
[ ] Three bids for building materials (must be on contractor’s company letterhead)

From Nov 2017 through Jan 2018, United Way of Brazoria County will be conducting a Community Outreach Blitz across Brazoria County.
